Request for Corporate Billing

If your company would like to receive a Net billing account, please fill in the information below.  

You will need the usual credit information:  

1. Bank information, including account #, Bank phone and address
2. three credit references
3. your physical business address
4. your accounts payable representative - name and direct phone number
5. A valid credit card to hold on file

We ask that your first purchase be done with a major credit card before being issued your billing terms.
